Position Function Summary
The Behavioral Health Provider, as a key part of La Familia Medical Center’s (LFMC) primary care treatment team, identifies, triages and manages patients with behavioral health problems within the primary care setting. In addition, the Counselor/Social Worker will provide skill training through psycho-education and patient education strategies and will develop specific plans for a patient’s behavioral health assessment, treatment and referral.
Essential Duties and Responsibilities
- Assists primary health care providers in recognizing and treating mental disorders, substance abuse and psycho-social problems;
- Assesses the clinical status of patients referred by primary care providers through brief consultative contacts. Assesses, treats and refers pediatric, adults and geriatric patients, individuals, couples and families;
- Works with the primary care team to treat and manage patients with chronic emotional and/or health problems efficiently and effectively. Provides consultations for all three clinics;
- Assists in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration;
- Assists in preventing relapse or morbidity in conditions that tend to recur over time;
- Develops patient care plans with patient and behavioral health team;
- Teaches patients, families and staff care, prevention, and treatment enhancement techniques;
- Identifies problems related to patient services and makes recommendations for improvement;
- Attends and participates in meetings and quality improvement activities as required;
- Responds to patient or co-worker complaints and works toward a positive resolution of any dispute;
- Assists LFMC staff and their families with behavioral health referrals;
- Serves as a member of site committees, as requested;
- Attends residency POLS, IPOLS, planning meetings as requested by supervisor;
- Attends provider meetings at the request of supervisor.
Education, Experience and Skills/Training
- Independently Licensed Clinical Social Worker (LCSW), required, from an accredited Master of Social Work program.
- Minimum three (3) years of experience in the field.
- Familiar with referral resources in our community.
- Skilled in use of short-term solution focused psychotherapy.
- Fluent in Spanish and English, strongly preferred.
